Significance of Human Papillomavirus in Head and Neck Cancers

© 2015 Harborside Press® Over 45,000 Americans are estimated to be newly diagnosed with cancer of the oral cavity and pharynx in 2015, and an estimated 8,650 will die of the disease (Siegel, Miller, & Jemal, 2015). It is well known that the combination of smoking and alcohol use increases the risk for developing head and neck squamous cell carcinoma (HNSCC). Head and neck squamous cell carcinoma is a general term that encompasses cancers of the nasal cavity, sinuses, lips, mouth, salivary glands, throat, and larynx. HNSCC has been considered to be a cancer predominantly seen in men of older age (National Cancer Institute [NCI], 2013). The current trends, deaths, and survival rates demonstrate a modest decrease in HNSCC from 1975 to 2006 (NCI, 2015). However, despite this general decrease, research demonstrates that oropharyngeal-specific cancers are rising (American Cancer Society, 2015; Gillison et al., 2012). Currently, there is evidence that tissue collected from patients diagnosed with head and neck cancer may be positive for human papillomavirus (HPV) DNA, particularly cancers of the oropharyngeal site (Mehanna et al., 2013). The oropharynx refers to the middle part of the throat behind the mouth, including the back third of the tongue, the soft palate, the side and back walls of the throat, and the tonsils (NCI, 2015). According to the Centers for Disease Control and Prevention (CDC, 2013), 79 million people have HPV, and over 14 million cases are newly diagnosed each year. This virus is associated with cancer of the cervix, vagina, penis, and anus. The exact incidence of HPV-positive HNSCC is unknown and could be largely misrepresented because current practice does not support testing all HNSCC for HPV status. Currently, the Surveillance, Epidemiology, and End Results Program (SEER) does not specify HPV status in statistics on oral cavity and pharynx cancers. However, according to the CDC (2014a), an estimated 72% of oropharyngeal cancers may be attributed to HPV infections, with more than 2,370 new cases diagnosed in women and nearly 9,356 new cases diagnosed in men every year in the United States.

Human papillomavirus status in head and neck cancer: the ethics of disclosure.
Human papillomavirus (HPV) is an emerging causative factor for squamous carcinoma of the oropharynx and perhaps other head and neck cancers. There is a great deal of uncertainty regarding the clinical significance and implications of HPV status in this patient population. As a result, there is no established protocol for informing patients of the potential link between viral infection and their...

BACKGROUND The neutrophil-to-lymphocyte ratio (NLR) has prognostic significance for many cancers, with higher values correlating with poor outcomes. The purpose of this study was to determine the prognostic significance of this inflammatory marker for patients with head and neck squamous cell carcinoma (HNSCC). METHODS Univariate logistic regression and multivariate Cox regression analyses we...
متن کاملHuman papillomavirus infection as a risk factor for squamous-cell carcinoma of the head and neck.
BACKGROUND Oncogenic human papillomaviruses (HPVs), especially HPV type 16 (HPV-16), cause anogenital epithelial cancers and are suspected of causing epithelial cancers of the head and neck. METHODS To examine the relation between head and neck cancers and HPVs, we performed a nested case-control study within a joint Nordic cohort in which serum samples were collected from almost 900,000 subj...
متن کاملHead and neck cancer--Part 2: Treatment and prognostic factors.
In this second of a two part series, we discuss recent advances in the management of cancers of the head and neck. We also discuss the important prognostic factors, including the importance of human papillomavirus (HPV) positivity in the newly discovered HPV related cancers of the head and neck. As before, we have used evidence from national guidelines, randomised trials, and level II-III studi...
متن کاملThe characteristics of human papillomavirus DNA in head and neck cancers and papillomas.
AIM To determine the prevalence, type, physical state, and viral load of human papillomavirus (HPV) DNA in cases of head and neck cancer and recurrent respiratory papillomatosis (RRP). METHODS The prevalence and type of HPV DNA was determined in 27 fresh frozen tissue specimens from patients with head and neck cancers and 16 specimens from 10 patients with RRP by MY09/MY11 and GP5+/GP6+ neste...
متن کاملClinical characteristics of patients with multiple potentially human papillomavirus-related malignancies.
BACKGROUND Human papillomavirus (HPV) is a causative factor in squamous cell carcinomas of the anus, penis, vagina, vulva, and head and neck, and adenocarcinoma of the cervix. We examined the demographics, clinical characteristics, and timing of multiple potentially HPV-related cancers in individual patients. METHODS One hundred forty-three patients were identified with 300 potentially HPV-re...
